Template #2: Evaluation/Measures of Effectiveness

This template can help an agency determine how to ensure that the goals identified in Template #1 are being met throughout the campaign. For each goal, identify one or more measures of effectiveness that can be used to determine if that goal is being met. Then identify tools or strategies that can be used to obtain the data and information needed to determine if the effectiveness measures were met. Appendix C provides a sample filled out version of this template.
